Long Term Medical Conditions (LTMC)

We operate dedicated Long Term Medical Conditions Clinics. This is an innovative approach our practice cares for patients with long term conditions like asthma, diabetes, stroke, hypertension, COPD and coronary heart disease. These clinics were set up to reduce the need for patients to make multiple visits to the practice to see the practice nurse and the doctor. The LTMC clinic is run by our Practice nurse with help from GPs.

NHS Health Checks

The NHS Health Check is for adults in England between the ages of 40 and 74. It will give you and your GP a clearer picture of your health, and help you take action to reduce your risk of heart disease, stroke, type 2 diabetes and kidney disease.

The NHS Health Check involves a few straightforward blood tests and some simple questions about your medical history. This will take around 20 minutes.

You’ll be given personalised lifestyle advice and support to help you lower risk of developing any vascular problem including heart attack and stroke. You will receive advice on how to get more physical activity into your daily routine, or how to eat a healthier diet.

BP MANAGEMENT

Blood Pressure Management

The nurses will monitor and perform the regular 6 monthly checks on patients with high blood pressure in accordance with the guidelines for the management of hypertension. At your check up the nurse will take BP and pulse; test urine specimen annually; check weight and advise; check smoking habit and advise.

We have installed a modern self-checking BP machine in waiting area. It is very simple to operate and could be used by patients. Please hand over the slip of the readings generated by machine to reception, so they can update your medical records.

Weight Management

Patients having problems with losing weight could be referred to weight management scheme. Please make appointment with your GP if you have problem of weight gain or unable to lose weight. If all your examination and investigations are negative, you could be referred to weight management programme.

The Weight Management Scheme is a partnership opportunity between Health professionals and slimming groups to provide patients with effective, lasting support for weight loss. This service is free. Please book an appointment with the practice nurse for full assessment so you can be referred.

BABY CLINIC

Baby Immunisation

The Child Health Authority will notify parents via post about their child’s immunisation appointment. We operate a dedicated child immunisation clinic with the practice nurse on every Wednesday morning.

Rochdale road medical centre follows the routine childhood immunisation programme. Each vaccination is given as a single injection into muscle of the thigh or upper arm.

Maternity & Midwifery

If you think that you could be pregnant, please do a pregnancy test using a pregnancy test kit available over the counter and in pharmacies.

You need to make an appointment to see one of our doctors when convenient. Once your pregnancy is confirmed you will need to see a midwife on a regular basis.

The district nursing team is responsible for providing the following services. You don’t have to see GP for these services.

  • Dressings
  • Wound checks
  • Removal of stitches
  • Leg ulcers

Bowel Cancer Screening

The Bowel Cancer Screening Programme (BCSP) aims to reduce bowel cancer mortality by detecting and treating bowel cancer, or pre-cancerous growths, at an early stage. It does this by looking for hidden blood in stool samples. FOBt kits are posted out by a Screening Hub every two years to the registered population aged 60 – 74 (aged 75+ can opt in by ringing free phone 0800 707 6060) to collect samples at home.

Physio Direct – Self-referral

Physio Direct is a self-referral service that allows patients to access physiotherapy treatment without seeing a GP first.

You must be 16 years of age or over and have musculoskeletal problems such as back pain, neck pain, painful joints and/ or general soft-tissue sprains and strains. This includes sports injuries, work related injuries and postural problems.

To access this service, please contact the surgery and reception will be able to give you the required form.
